Service Description
This visit lasts 1-1.5 hours and includes a thorough health history, an observation and assessment of a nursing session, a pre- and post-feed weight check, a customized care plan, a report to your health care provider(s), and access via telephone, text, and/or email for follow up questions for 2 weeks. Prices reflect travel time and fuel costs incurred to travel to your home. Price may vary if your home address is outside our travel radius. In the event you need to change or cancel your booking, our cancellation policy is as follows: -To cancel or reschedule, please contact us 48 hours in advance to receive a full refund. -If booking is cancelled less than 24 hours in advance prior to the scheduled appointment time, 50% of the consultation fee is payable at time of cancellation. -If given appropriate notice, we are more than happy to reschedule. We will reschedule appointments once at no cost to you. If you have rescheduled once already and you cancel the second time, this is considered a cancellation and fees will apply at the time of cancellation. -Reversal of monies paid will incur a 3% administration fee (regardless of reasons for cancellation). Please allow up to 10 days for monies to appear in your account. -Anchored Wellness, LLC reserves the right to cancel or reschedule appointments if required Intake Questionnaires/relevant consents are not completed prior to scheduled appointment time. We reserve the right to use our discretion depending on circumstances. If an appointment is cancelled at the last minute, this can be difficult to fill for another client who we may have already turned away. Thank you for your cooperation and understanding.
